Welcome
to my website.

You can get a first impression here.
However, detailed personal advice is essential before any medical, but above all every aesthetic intervention!

At the beginning of the doctor-patient contact there is extensive advice. This is followed by an analysis of the initial situation, I explain the surgical method in detail, comment in detail on the risks and the healing process and can then also make statements about the chances of success.

Only then should you make the decision about a possible operation. The importance of detailed and honest advice can only be judged by whoever gets it!

AESTHETIC FACIAL SURGERY

Treatments

  • NOSE CORRECTION

AESTHETIC CORRECTION OF THE OUTER NOSE

The nose consists of a bony and cartilaginous scaffold that is covered by a skin coat and lined with mucous membrane. The operation can be performed in almost all cases from the inside of the nose so that no visible scars occur. It is necessary to lift the skin coat from the bony and cartilaginous nasal skeleton. This leads to swelling and discoloration. While the latter usually subside in two, at the latest three weeks, low swelling conditions may persist for some months. Swelling and thickening, even one-sided, are among the most common symptoms that require treatment after surgery. Particularly affected is the area above the tip of the nose, especially with very thick skin. On the other hand, with thin skin, in rare cases externally visible constrictions can be caused by the scar.

Especially in patients prone to circulatory disorders, the skin of the nose or nose tip may show a reddish or bluish discoloration after surgery.
Crooked rabbits are particularly troubled because they tend to return to their old position after straightening. Here, the nasal septum must be mitkorrigiert to counteract this tendency.

If the nose is to be made smaller externally, it can not grow inside. Under certain circumstances, this must be reserved for a second, usually much smaller intervention.
Just as little as two sides of the nose and two nostrils are absolutely symmetrical before surgery, they will often not be the same as a mirror after surgery. With slight, barely noticeable asymmetries one must expect.

In the case of a saddle nose, cartilage is used to build up the nasal septum, the auricle, the rib cartilage or foreign cartilage. Unfortunately, while these implants heal properly in most cases, they are also known to be broken down or even repelled.
Very rare events in all nasal surgeries are increased rebleeding or infection. Just as rare are pressure points under the plaster cast after the surgery, because they usually show pain in time.

Occasionally, there is a slight fall in the tip of the nose after the operation, less often a connective tissue-like swelling above the tip of the nose.
While there is not a slight nose correction in the true sense, many nasal intrusions cause the surgeon extraordinary difficulty. For the patient itself, experience has shown that it is impossible to judge them correctly. As you can see from the possible complications listed above, it is important for the surgeon to control the dynamics of the postoperative healing process. The surgical result is only secured by careful post-treatment.

Extreme cold and sunshine should be avoided for about six months.
The procedure is performed under general anesthesia by a specialist anesthesiologist.

If you are interested in an intervention, you can arrange a meeting appointment at the telephone number given above. You then have the opportunity to discuss the procedure as well as your wishes and expectations in detail with the surgeon. This will inform you in detail about the possibilities and limitations of surgical treatment. This explanation includes not only the nature, meaning and consequences of the procedure in its essential features, but also the reference to the possible complications, as they u. a. also described above. You also have the opportunity to visit the clinic.

  • AURICULAR CORRECTION

OTOPEXY - FOR A NATURAL RESULT

Protruding ears often disturb self-esteem, especially in children. A correction of the ears is in such cases not only of aesthetic value but also psychologically sensible.
But even adults sometimes have the need to put their ears.

The aim of the surgery is to improve the angle between the head and ear and to obtain or achieve a natural auricle shape. For this purpose, various techniques are available that would be too confusing to list in detail. Basically, the procedure is performed from the back of the ear, so that the inevitable scar is not visible. The operation lasts about 20 to 30 minutes per ear and is very good for adults to perform in local anesthesia, possibly combined with a twilight sleep. For a few days, a headband-like bandage must be worn afterwards.
Just as little as two ears are absolutely symmetrical before surgery, they often will not be the same as a mirror after surgery. With slight, barely noticeable asymmetries one must expect.

Pain after a ear donning plastic are very rare and easy to control by mild painkillers.
To avoid infections, an antibiotic is administered during the operation.

Overall, the complication rate in this procedure is very low and the postoperative result convincing.

If you are interested in an intervention, you can arrange a meeting appointment at the telephone number below. You also have the opportunity and the clinic to visit.
At the beginning of the doctor-patient contact is a detailed consultation. In this an analysis is carried out e.g. the nose, I explain in detail the operation method, take a detailed position on risks and the healing process and can then also make statements on the prospects of success. Only then should you decide on a possible operation.

How important a detailed and honest consultation is can only judge who gets it!

  • EYELID LIFT

LOWER EYELID SURGERY | UPPER EYELID SURGERY

    LOWER EYELID

Dependend on heredity or age, a lot of people tend to have pouches with deep sulci and wrinkles in the lower eyelid area.
This causes a faded and sad looking facial expression. The pouche consists of bulgy fat tissue from the eye socket, which could not be hold back to the socket due to weakened band structures.

    UPPER EYELID

Over the years, some people get a histoid excess between the upper eyelid and the eyebrow caused by a reduce of the skin’s elasticity. This is known as a ’hooded eye’. which can, depending on the form, limit the visual field. It usually causes a heavy, tired facial expression which is not matching with the sense of life.

Women often moan about not being able to makeup their eyelashes since the mascara always smudges at the skin excess on the upper eyelid.
The first signs of the aging process usually occur at the eyelids. The aim of the operation is to regive your fresh-faced look by removing the excess skin.

The operationcuts run on the upper lid along the natural wrinkle, on the lower eyelid circa 2mm under the edge of the eyelid. The cuts reach further than the outer corners of the eye do, they end in a laughter line when operating on the lower eyelid. Where cuts are made, scars are created. The development of the scars depend –as always- on the skin’s tendency to heal, but usually has a very positive outcome. That means, rednesses and swellings of the scars as well as of the surrrounding skin are usually gone after a little time. The scar on the upper eyelid is located on the skin envelope fold, so just visiblewith a closed eye, the lower eyelid’s scar can, if necessary, be covered with make-up.

As with all operations, infections and after-bleedings are possible, bu tvery rare.

Like two eyes are very unlikely to be absolutely symmetric prior to the operation, they will be afterwards. You have to expect minimal, slightly visible asymmetrics.
A rare but possible incident can be a shortening of the lower eyelid, oftenly caused by scarry contraction process of the skin or muscles of the eyelid.

A blue coloration of the skin always occurs after the operation. Eyes which tended to redness and dryness, can be enhanced after the operation, but most of the time just temporary.

At the beginning of the doctor-patient-contact is a detailed consulting, in which an analysis for example of the nose follows, I explicitly explain the method of the operation, tell you in –depth about risks and the process of healing and after all that, I can give a statement about the chance of success. Only then, after all that, you should make a decision about a possible operation.

Only patients who get a detailled and honest consulting know how important it is!

  • WRINKLE SMOOTHING

WRINKLES AND CRINKLES – DEFINING OR ANNOYING?

The individual countenance creates the facial wrinkles. Those wrinkles influence the character and the general physical appereance of every single face.
Depending on the wrinkle’s form and location, a thoughtful, sad, angry or tired facial expression can develop. To correct those wrinkles and scars, an implanation of different compounds is available, besides the operative treatments.

    BOTOX®
Botox is used by eye doctors to eliminate convulsions of the eyelid for many years. Botox is a crystalline proteincomplex consisting of Clostridium botulinum Toxin Type A and several carrier substances. It blocks the transmittance between the nerves by preventing the release of Acethylcholin und causes reversible paralysis of the voluntary muscles.
Frown wrinkles, forehead wrinkles and crow’s feet wrinkles are removed with Botox since 1994.

Specific injections with 0,1ml Botox (a hundred times diluted and therefore not poisonous!) to the muscles lead to a temporary quiescence without having a functional impairment. The impact happens after 3-5 days after the treatment. The injection sites should not get in contact with water for 8 hours afterwards.

A slight pressure to the head can occur after the treatment. A minimal effusion of blood is very unusual, but can be covered by make-up anyways. Botox allergies are not known, an infection effectively impossible.
If you are interested in an operation, please contact us under the phone number given above and arrange and appointment with us. You will have the possibility to talk about your wishes and expectations, as well as about the procedure in general, directly with the operator. The surgeon will tell you about the potentialities and limits of that operative therapy. This enlightenment contains not just type, meaning and consequences of the operation but also the consideration about the possible complications like you have seen above. You also have the oppurtinity to take a closer look on the clinic.

The success of the treament lasts for 4 to 6 momths, often even longer.

If you are interested in an intervention, you can make an appointment at the telephone number given above. You will then have the opportunity to discuss the procedure as well as your wishes and expectations in detail with the surgeon. Ear correction in Mülheim by surgery

Ear correction is one of the most popular operations in facial aesthetic surgery, especially for men. Big ears, also known medically as otopexy, has a long history. As early as 1881 a US surgeon performed a so-called otoplasty for the first time. In 1885 the German doctor Johann Friedrich Dieffenbach also described the surgical ear correction. The purpose of surgical big ears is first of all to remove an existing asymmetry or simply sticking out the ears, which can disrupt the overall appearance of the face and lead to psychological stress. But how exactly does the big ears work? And what do you have to pay attention to after the operation?

The ear correction procedure and important information
Ear correction can be performed from the age of six. From this age onwards, the human ears have grown to a good 90 percent and do not change much later on. Although big ears is a standard treatment within aesthetic plastic surgery, patients interested in this procedure should pay attention to a few important points. For example, you should refrain from taking painkillers, blood-thinning and antipyretic drugs for at least two weeks before the operation.
There are many different types of big ears surgery. In the suturing technique, for example, the back surface of the auricle is exposed and the ear cartilage is brought into position by pulling threads. With the combined suture-cut technique, however, the auricle is reshaped through a small incision behind the ear and, if necessary, part of the ear cartilage is also removed.

What should you watch out for after big ears?
Due to the sensitivity of the human ear, it should be protected by a sterile head bandage or a removable bandage for at least three to four days after the ear correction. After removing bandages, it is advisable to wear a special headband while sleeping for about six weeks. As with any operation, complications such as bruising or wound healing disorders cannot be ruled out when big ears. However, when performed by an experienced specialist, big ears are extremely low-risk.

Various options for wrinkle injection

The wrinkle injection is one of the most popular non-invasive (i.e. non-surgical) aesthetic treatments. In comparison with aesthetic plastic surgery, wrinkle injections are very popular in Essen and the surrounding area and, measured in terms of the number of treatments, is only surpassed by breast augmentation and eyelid tightening in Germany. In many cases, wrinkle injection is a gentle alternative to surgical procedures such as the facelift, or it can at least significantly delay the need for a major intervention.
Filler for injecting wrinkles in food.

While a few years ago a wide variety of agents were used for wrinkle injections, including non-absorbable substances such as silicone, today hyaluronic acid has become the standard for filler treatment. Hyaluronic acid is also used in other medical areas and occurs naturally in the human body, which is why it is particularly well tolerated. So-called fillers based on hyaluronic acid are available today in a wide variety of consistencies in order to do justice to the various areas of application. There are preparations that have been specially developed to correct small wrinkles in the area of the mouth or fillers that are suitable for plumping up sunken areas of the face. Hyaluronic acid has a strong water-binding effect and an experienced specialist can successfully even out pronounced wrinkles through targeted injections. The effect of a filler treatment with hyaluronic acid lasts for about 4 to 6 months, in some cases significantly longer.

Botulinum toxin as an alternative to wrinkle injections
With the so-called botulinum treatment, the aesthetically active specialist has another proven means of non-invasive wrinkle reduction. Even if wrinkle treatment with botulinum toxin type A (better known under the brand name Botox) is a very common injection therapy, it is not actually a wrinkle injection. Because the also absorbable botulinum toxin is not used to fill up wrinkles, but to Temporarily numbing the muscles that cause expression lines to appear. Even with the so-called botox treatment, the effect lasts for about 4 to 6 months. If necessary, the treatment can be repeated without any problems.

Eyelid lift in Mülheim: good education and realistic expectations

For many patients, an eyelid lift in Essen represents an opportunity to get rid of unloved "bags under the eyes" and "drooping eyelids" and to regain a fresher and more youthful facial expression. Since the skin on the eyelids has a particularly thin structure, the signs of the times are usually the first noticeable there. Patients then often radiate a feeling of exhaustion outwardly, which does not correspond to the actual feeling. It can even happen that the field of vision is restricted if the "drooping eyelids" are particularly pronounced. With an eyelid lift in Essen, those affected have the opportunity to regain a more lively facial expression without changing their basic appearance.

Who is an eyelid lift in Essen suitable for?
Patients interested in an eyelid lift in Essen often suffer from sagging and wrinkled skin around the eye area. The excess tissue on the upper eyelid is mainly caused by the loss of skin elasticity. The so-called eye bags on the lower eyelid are formed as a result of the natural aging process or are hereditary. They consist of swollen fatty tissue in the eye socket, which can no longer be held by the weakened ligament structures of the inside of the eye. During an eyelid lift in Essen, the doctor removes the excess skin and fatty tissue and thus gives the eyes a more alert appearance.

Eyelid lift in Essen - the consultation
Before patients decide to have an eyelid lift in Essen, they should always have a detailed consultation with their doctor. Those affected have the opportunity to openly communicate their questions and wishes to the doctor and are also informed about possible risks. In addition, a detailed examination of the eyes takes place in order to rule out possible previous illnesses. Patients are also advised not to use cosmetics or creams around the eyes on the day of the operation. Even if the eyelid lift in Essen is one of the low-risk procedures, as with any operation there is a risk of infection and bleeding. Patients should also be aware that two eyes never look the same and that slight asymmetries can occur even after the eyelid lift. In addition, the doctor cannot reverse the natural aging process with the procedure, but he can very well adapt your eye expression to your youthful attitude to life.

How does an eyelid lift work in Essen?
During an eyelid lift in Essen, the doctor removes the excess skin and fatty tissue. The procedure is usually performed under local anesthesia and takes about half an hour per lid. On the upper eyelid, a cut is necessary along the natural crease of the eyelid; on the lower eyelid, the cut is about 2mm below the edge of the eyelid. The cuts run past the corner of the eye on both sides and end in a laugh line on the lower eyelid. Of course, this creates scars that patients should be aware of beforehand. How the scars heal depends on the skin's healing tendency, which is different in every person. In the eye, however, the healing process is generally very positive, so that patients should not be too concerned if the scar begins to redden and swell. In addition, the scar can only be seen when the eye is closed due to its location on the crease.

What do patients have to consider after an eyelid lift in Essen?
Immediately after the eyelid lift in Essen, it is advisable to rest for about two hours with the upper body slightly lifted under medical supervision so that any complications that may arise can be treated immediately. As soon as the anesthetic wears off, you may experience slight pain. To support the healing process, it is advisable to cool with cold compresses. The swelling and bruising usually go away after about two weeks. In the first few weeks after the eyelid lift in Essen, the doctor recommends wearing dark glasses and avoiding direct sunlight. Patients should also initially refrain from physically demanding activities, sport and going to the sauna.

With an eyelid lift, an experienced surgeon can give a face a significantly fresher, younger look. Accordingly, eyelid correction is equally popular with male and female patients. The human eyelid fulfills important functions that should not be overlooked when performing an eyelid lift.

What to consider when doing an eyelid lift: Functions of the eyelid
The main purpose of the human eyelid is to protect the eye from contact and other harmful environmental influences. By blinking (blinking), it also ensures continuous moistening of the cornea and dermis. Without this supply of tear fluid, the outermost layers of the eye would dry out and be damaged in the long term. Therefore, a functioning eyelid is also important for maintaining good eyesight. In addition, the eyelids, as part of the eye area, make a significant contribution to a person's facial expressions. Eyelids can express certain feelings such as joy and sadness, but also horror or fatigue.

When doing an eyelid lift, these various functional aspects must be taken into account.

Bags under the eyes and drooping eyelids: signs of aging of the eyelids
In the course of life, the human face changes continuously. With advancing age, the elasticity and firmness of connective tissue and skin in particular decrease, causing many areas of the face to sag or sag. The aging process does not leave the eyelids without a trace. Here, sagging is particularly noticeable in the form of bags under the eyes and drooping eyelids. These signs of the times would not be a reason for an eyelid lift in and of themselves. However, excess tissue in the upper eyelid area (drooping eyelids) can sometimes impair the visual field. In this case, an eyelid tightening is not only indicated for purely aesthetic reasons, but also makes sense due to functional limitations.

When a nose operation may be medically necessary

Many people associate a nose operation primarily with the correction of the nose based on aesthetic wishes and ideas. And in fact, the aesthetic nose operation is one of the most common so-called cosmetic operations worldwide. However, only those who suffer from actual functional complaints in connection with their nose are usually aware that a nose correction can also be medically necessary. In this case, the specialist in ear, nose and throat medicine will explain whether a nose operation can solve the problem.

Aesthetic wishes as a reason for a nose operation
Often a nose operation is based on aesthetic desires. Quite a few people are bothered by a nose that is too large and that does not match the other proportions of the face. A so-called hump nose is also the reason that many people are interested in nose surgery. Thanks to proven techniques, an experienced nasal surgeon is able to correct the nose in terms of size, length, shape or width. Aesthetic correction of the nose is often the step towards a happier life for patients who have suffered from the shape of their nose for a long time.

Functional reasons for a nose operation
In many cases, functional problems, particularly difficult nasal breathing, actually make nasal surgery a medically necessary procedure. Reasons for problems with nasal breathing include a crooked nasal septum or enlarged turbinates. Blockages of the nose due to malformations, tumors, polyps or the consequences of injuries are less common. During a detailed examination, the specialist determines the causes of the breathing problems and recommends the appropriate therapy - for example a nose operation.

Advice on nose surgery in Essen - with Dr. Holger Marsch
Whether a nose operation is only indicated aesthetically or whether there is a functional, i.e. medical reason for the operation, can be clarified in a comprehensive consultation. As an experienced ENT doctor with a focus on nose surgery, Dr. Holger Marsch takes functional factors such as optimal nasal breathing as a matter of course in every nose operation. For him, providing detailed advice to his patients also includes informing them about possible medical indications for a nose correction.

From ear correction to filler treatment: aesthetic facial surgery worldwide

Aesthetic facial surgery is at home all over the world and is enjoying increasing popularity. Most of the operations are carried out in the USA and Brazil, and many aesthetic operations are also carried out in Asian countries such as Japan and South Korea. This was the result of a study of surgical and non-surgical interventions worldwide by the International Society for Aesthetic Plastic Surgery (ISAPS), which was published in 2015.

Brazil leads the ranking in facial aesthetic surgery
According to the study, over 500,000 operations in the field of aesthetic facial surgery took place in Brazil, which corresponds to a global share of almost 12%. The USA follows closely behind, accounting for 10.4% of all facial surgeries worldwide. Germany ranks sixth with over 100,000 interventions.

Almost 250,000 ear corrections worldwide
In the field of ear correction, Brazil ranks first with over 45,000 procedures, followed by the USA with more than 13,000 procedures. Mexico ranks third worldwide with over 10,000 operations. In Germany, the ear correction was performed almost 6,000 times in 2014, which corresponds to seventh place in the study.

Mostly female patients undergoing rhinoplasty
Almost three-quarters of all patients, 72.3%, were female when it came to rhinoplasty, but rhinoplasty only ranks sixth among all operations for women and second for men. Rhinoplasty was the most common type of procedure in South Korea, with over 100,000 procedures. In 2014, more than 12,000 operations took place in Germany, which corresponds to 1.4% of all nose corrections in the world.

Most popular filler treatment in the US
Among the non-surgical procedures, the filler treatment with hyaluronic acid and wrinkle injections with botulinum toxin are particularly popular. 28% of all wrinkle injections were done in the USA, followed by Brazil (7.4%) and South Korea (5.7%). The order of the filler treatment is the same, but in Germany, with a share of 3.6%, these are more popular than wrinkle injection (2.7%).

Rhinoplasty in Mülheim: what should be considered before the operation?

The rhinoplasty is one of the most demanding operations in plastic surgery. The anatomy of the nose is complex and the olfactory epithelium (a layer of cells in the top of the nasal cavity) connects the nasal cavity and the brain directly. Therefore, only experienced specialists should perform rhinoplasty. Dr. Holger Marsch, specialist in ear, nose and throat medicine with additional qualification in plastic operations, has many years of experience in nose corrections. In his practice in Essen, he performs nose corrections almost every week.

Comprehensive advice before each rhinoplasty
Well-informed patients are the be-all and end-all for the ENT specialist in Essen. Before each rhinoplasty, he advises and examines his patients in detail, explains the risks and possible complications. Despite the difficult surgical technique, the risks associated with surgical rhinoplasty are rather low. However, as with any operation, wound infections and intolerance, for example, are possible. Dr. Marsch recommends that his patients write down their wishes and questions before the consultation. "The better informed our patients are, the fewer uncertainties and fears they feel, even before a nose operation," says the Essen specialist. So it is important that patients do not take aspirin or other antipyretic, anti-rheumatic drugs or high-dose vitamin E before a rhinoplasty, because these preparations delay blood clotting.

Thorough examination before rhinoplasty
A thorough examination of every patient, especially of the nose and face, takes place prior to the rhinoplasty procedure. The examination is particularly important in order to rule out functional disorders of the nose and to select the correct surgical method for each individual. The breathing function of the nose should be checked, a smell test carried out and - if necessary - an X-ray of the paranasal sinuses should be made.

From hump nose to crooked nose: reasons for a nose correction

Rhinoplasty is one of the most common operations in facial aesthetic surgery, but also in plastic surgery as a whole. The fact that rhinoplasty is so popular in Essen and beyond is mainly due to the fact that the nose is so important for the effect and charisma of a person due to its central location on the face. For those who are dissatisfied with their nose, nose surgery is a good and low-risk option.
But which characteristics can be changed in a nose operation?

Rhinoplasty in Essen: Various changes in shape
In the course of a nose operation, the shape of the nose can be corrected in many ways. Since both bone and cartilage material are processed during the procedure, the height, width, length and shape of the nose can be changed. The focus of the operation can be on the bridge of the nose as well as the ala or the tip of the nose. When choosing a surgeon, however, patients should always make sure that they are experienced surgeons who pay attention not only to aesthetic but also to functional aspects during the rhinoplasty. Because hardly anyone will be happy about a supposedly more beautiful nose if their nasal breathing is impaired.

Indications for a nose job in Essen
In medicine, an indication is the reason for a certain measure, i.e. a treatment or an operation. There are also a number of typical indications for rhinoplasty. This includes, for example, the so-called hump nose, in which the nasal bone is marked by a clear hump or hook. Another example is the saddle nose, recognizable by a heavily sunken bridge of the nose. The crooked nose, on the other hand, is - unsurprisingly - a clearly crooked or crooked nose. Other indications are the bulbous nose (also known as rhinophyma) and the balloon nose.
